OP amp calculation help.

Hi guys, i've designed this OP amp circuit, but i cant remember how i calculated the 1.034 Volts, and the resistors. Can anyone help? https://www.circuitlab.com/circuit/a4n564/help/

Thanks for the reply. I figured the formula to be :

V(out) = (1 + R2/R1) * V(+) - R2/R1 * V(-)
